Job Summary
To lead complex patient journeys in Substance Use Disorder care, the full-time Senior Clinical Care Navigator will manage high-acuity cases, develop scalable workflows, and collaborate with clinical and operational teams to ensure optimal patient outcomes in a remote setting.
Key Responsibilities
- Own the end-to-end success of the Substance Use Disorder service line, ensuring coordinated and timely patient care
- Manage complex care coordination scenarios, facilitating seamless patient transitions through their care journey
- Coach and mentor junior team members on best practices and workflow strategies
Required Qualifications
- 4+ years of clinical experience in Substance Use Disorder and Behavioral Health with relevant licensure (LCSW, LCPC, LSW, or LPC)
- At least 3+ years of direct patient care or coordination experience
- Substance Use Disorder certification is strongly encouraged
- Preferred: Spanish speaking and direct experience in broader behavioral health
