Licensed Clinical Care Navigator

Job Summary

Providing clinical support and resources, the full-time Licensed Clinical Care Navigator will work remotely from Tuesday to Saturday, 9am-5pm ET, assisting members with mental health, substance use, and relationship concerns while conducting assessments and coordinating care.

Key responsibilities

• Deliver in-the-moment clinical support and referrals through outbound calls, with some inbound crisis line support
• Conduct risk assessments and provide crisis support and safety planning for high-risk members
• Collaborate with multi-disciplinary teams to ensure a seamless member experience and effective care coordination

Required qualifications

• Active LCSW, LMFT, LPC, or licensed psychologist credential
• Minimum of 2 years of direct clinical care experience
• Expertise in crisis assessment and safety planning
• Knowledge of crisis support techniques, including CBT and motivational interviewing
• Proficiency in using various computer platforms and technology
