Licensed Clinical Care Navigator

Job Summary

Providing clinical support and resources, the full-time Licensed Clinical Care Navigator will work remotely from Sunday to Thursday, 9am-5pm ET, engaging with members on mental health and crisis issues while coordinating care and referrals.

Key responsibilities

• Deliver in-the-moment clinical support and psychoeducation through outbound calls, including some inbound crisis line assistance
• Conduct risk assessments and provide crisis support, safety planning, and follow-up for high-risk members
• Collaborate with multi-disciplinary teams to ensure a seamless member experience and promote effective care

Required qualifications

• Valid LCSW, LMFT, LPC, or licensed psychologist credential
• Minimum of 2+ years of direct clinical care experience
• Expertise in crisis assessment and safety planning
• Knowledge of crisis support and various clinical interventions such as CBT and motivational interviewing
• Proficiency in using technology and various computer platforms
