Licensed Clinical Care Specialist

Job Summary

Working remotely, the full-time Licensed Clinical Care Specialist will assess Medicare, Medicaid, and Group Account members, create actionable care plans, and coordinate care for chronic conditions while promoting health goals through a Whole Person Care approach.

Key responsibilities

• Telephonically assess members and develop measurable care plans to guide progress toward health goals
• Use nursing judgment to manage acute situations and facilitate safe transitions of care
• Collaborate with a multi-disciplinary team to address members' physical, environmental, and psycho-social health issues

Required qualifications

• BSN or MSN degree in nursing or equivalent
• Minimum of 3 years of clinical nursing experience with a focus on adult chronic conditions care management
• Active Compact nursing license and individual licenses in non-compact states
• Fluency in Spanish and English
• Experience with motivational interviewing and knowledge of MCG or CMS guidelines
