Licensed Clinical Case Manager

Job Summary

A company is looking for a Clinical Case Manager Behavioral Health DSNP.

Key Responsibilities

• Conduct comprehensive assessments of members' needs and eligibility for services
• Perform crisis intervention and coordinate care with clinical providers
• Develop care plans utilizing clinical judgment to address member issues and improve outcomes

Required Qualifications

• 3 years of direct clinical practice experience post-master's degree
• Knowledge of mental health and substance abuse disorders
• Experience using Motivational Interviewing and technological skills
• Valid unrestricted independent professional behavioral health clinical license
• Must be available to work Monday through Friday, 8:00am to 5:00pm
