Required Qualifications

• Master's degree in social work (MSW)
• Minimum of five (5) years of clinical experience in diverse healthcare settings
• Strong background in care management and chronic condition management
• Ability to work independently in a remote environment
• Comfort with technology and multiple EMR systems
