Job Summary

A company is looking for a Clinical Pharmacist to work remotely.

Key Responsibilities

Conduct medication consults via telephone, ensuring quality metrics are met
Identify and address medication adherence barriers in collaboration with patients and providers
Support patients in medication therapy initiation and maintenance, providing education and optimizing adherence

Required Qualifications

Ph.D. or Doctorate in Pharmacy from an accredited institution
Current unrestricted pharmacist license in the practicing state
1+ years of clinical pharmacy practice experience
Experience with Medication Therapy Management (MTM) and Comprehensive Medication Reviews
Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint)
