Licensed Clinical Pharmacist

Job Summary

To support a growing medication therapy management program, the full-time Licensed Clinical Pharmacist will perform comprehensive medication reviews, collaborate with clinical staff, and conduct telephonic consultations with medical providers while working remotely.

Key responsibilities

• Perform and document member-specific comprehensive medication reviews (CMRs) adhering to productivity and quality standards
• Drive medication interventions to ensure adherence to evidence-based medication guidelines among medical providers and members
• Collaborate with clinical care teams to integrate medication management into overall patient care and provide training on MTM resources

Required qualifications

• Bachelor's degree in pharmacy or PharmD
• Active pharmacist license and residency in the state of position
• 3+ years of experience practicing as a pharmacist, preferably in managed care or clinical settings
• PGY1 residency in ambulatory care, managed care, or related clinical field (preferred)
• BPS board certification in pharmacotherapy, geriatrics, ambulatory care, or related clinical field (preferred)
