Job Summary
Providing telephonic consultations, the full-time Licensed Clinical Pharmacist will educate patients on specialty medications, perform medication reconciliation, and manage disease states while working remotely.
Key responsibilities
- Conduct patient education consultations focusing on disease management and medication administration
- Document clinical interventions and maintain compliance with internal policies and regulatory standards
- Interpret prescriptions and communicate with healthcare providers to ensure accurate medication processing
Required qualifications
- Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree
- Current, unrestricted pharmacist license in state of residence
- At least two years of experience as a practicing licensed Pharmacist preferred
- Strong clinical background required
- Experience or understanding of Specialty Pharmacy medications desirable
