Licensed Clinical Pharmacist

Job Summary

Providing telephonic consultations, the full-time Licensed Clinical Pharmacist will educate patients on specialty medications, perform medication reconciliation, and manage disease states while working remotely.

Key responsibilities

• Conduct patient education consultations focusing on disease management and medication administration
• Document clinical interventions and maintain compliance with internal policies and regulatory standards
• Interpret prescriptions and communicate with healthcare providers to ensure accurate medication processing

Required qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree
• Current, unrestricted pharmacist license in state of residence
• At least two years of experience as a practicing licensed Pharmacist preferred
• Strong clinical background required
• Experience or understanding of Specialty Pharmacy medications desirable
