Licensed Clinical Pharmacist

Job Summary

Providing telephonic consultations to patients, the full-time Licensed Clinical Pharmacist will manage medication reconciliation, screen for drug interactions, and offer side effect management while working remotely.

Key responsibilities

• Conduct patient education through telephonic consultations focused on disease state management and medication administration
• Document patient consultations and interventions in pharmacy software, ensuring compliance with internal and external regulations
• Interpret and evaluate prescriptions for specialty medications, contacting providers for clarifications and expediting order processing

Required qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree
• Current, unrestricted pharmacist license in state of residence
• At least two years of experience as a practicing licensed Pharmacist preferred
• Strong clinical background required
• Experience and/or strong understanding of Specialty Pharmacy medications desirable
