Job Summary
Providing telephonic consultations to patients, the full-time Licensed Clinical Pharmacist will manage medication reconciliation, screen for drug interactions, and offer side effect management while working remotely.
Key responsibilities
- Conduct patient education through telephonic consultations focused on disease state management and medication administration
- Document patient consultations and interventions in pharmacy software, ensuring compliance with internal and external regulations
- Interpret and evaluate prescriptions for specialty medications, contacting providers for clarifications and expediting order processing
Required qualifications
- Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree
- Current, unrestricted pharmacist license in state of residence
- At least two years of experience as a practicing licensed Pharmacist preferred
- Strong clinical background required
- Experience and/or strong understanding of Specialty Pharmacy medications desirable
