Licensed Clinical Pharmacist
Job Summary
Seeking a full-time Licensed Clinical Pharmacist fluent in Cantonese/Mandarin to work remotely, who will manage chronic medication therapy, conduct comprehensive medication reviews, and collaborate with interdisciplinary teams to enhance patient care and medication adherence.
Key responsibilities
- Provide personalized care plans and educational materials to help patients manage chronic conditions
- Conduct comprehensive medication reviews and resolve drug therapy issues to optimize patient outcomes
- Drive quality pharmacy-related clinical measures to achieve "5-Star" performance and address therapy gaps
Required qualifications
- 3 or more years of clinical experience in direct patient care
- State licensure in good standing in at least 5 states, including Alabama, Florida, Georgia, or others as needed
- Residence in the Mountain, Pacific, or Hawaiian time zones or ability to support work during those hours
- Fluent proficiency in Cantonese/Mandarin
- Experience with collaborative pharmacy practice agreements preferred
