Licensed Clinical Services Coordinator

Job Summary

Supporting patient outreach and care coordination, the full-time Clinical Services Coordinator will engage patients in population health programs, manage scheduling, and collaborate with interdisciplinary care teams in a remote setting.

Key Responsibilities

• Conduct proactive patient outreach for wellness visits and preventive care initiatives
• Coordinate services across various healthcare sectors and assist patients in navigating healthcare resources
• Collaborate with care teams to improve patient engagement and reduce care gaps

Required Qualifications

• 3 years of experience in care coordination or case management
• Bilingual in Spanish and fluent in medical terminology
• Experience in safety-net or public health settings, such as FQHCs or community mental health centers
• Current state licensure as an RN, LPN, LSW, or LCSW is a plus
• Knowledge of Medicaid and Medicare populations and value-based care is desirable
