Licensed Clinical Social Worker

Job Summary

A company is looking for a Licensed Clinical Social Worker (Population Health).

Key Responsibilities:

• Conduct thorough biopsychosocial assessments to determine patient needs and appropriate interventions
• Provide evidence-based individual, group, and family therapy, and develop personalized care plans
• Coordinate care and services for patients, collaborating with multidisciplinary teams and maintaining documentation

Required Qualifications:

• Master's Degree in Social Work from an accredited school
• 2 years of clinical experience in care management or behavioral health
• Experience in social work counseling with complex psychiatric or substance abuse disorders preferred
• Licensed Clinical Social Worker upon hire
• Less than 1 year of experience in health coaching and motivational interviewing techniques preferred
