Job Summary

Empowering patients through personalized psychosocial support, the full-time Licensed Clinical Social Worker will provide counseling, conduct case management, and lead group sessions, all in a remote setting.

Key responsibilities:

• Provide case consultation and collaborate with interdisciplinary care teams
• Conduct case management, including assessing patient needs and coordinating care
• Lead group sessions focused on psychoeducation, stress management, and mindfulness

Required qualifications:

• LCSW license with licensure to practice in NY, FL, and MI (open to PA, LA, OH)
• 2+ years of experience as an LCSW
• Bachelor's and Master's degrees required
• Proficiency in using telehealth platforms and electronic health record (EHR) systems
• Experience working with patients in a clinic setting preferred
