Licensed Clinical Social Worker

Job Summary

To support a growing interdisciplinary team, the full-time Licensed Clinical Social Worker will provide integrated medical, behavioral, and social care to patients with complex healthcare needs through virtual and telephonic visits while working remotely within the PST time zone.

Key responsibilities

• Support the behavioral health needs of an assigned patient caseload using evidence-based models of care
• Collaborate with prescribing providers to manage psychotropic medication and monitor treatment adherence
• Document patient encounters and facilitate treatment plan changes based on patient progress and needs

Required qualifications

• Master's degree in Social Work
• Current independent licensure in California or Washington
• 2+ years of prior healthcare or behavioral health experience
• Geriatric care experience highly desired
• Proficient in Microsoft Office Suite and EMR documentation
