Licensed Clinical Support Coordinator

Job Summary

A company is looking for a Clinical Team Support Coordinator to support clinical care teams in managing patient care.

Key Responsibilities

• Provide clinical support for assigned care teams
• Update clinical records and address customer concerns
• Audit clinical records for compliance and assist in triaging patient visits

Qualifications

• Must hold a valid Practical Nurse or Registered Nurse license
• Minimum of two years of experience as an LPN/RN in a medical/surgical or home care setting preferred
• Ability to use sound clinical judgment and document accurately
• Must be empathetic, professional, and organized
• Proficient in Microsoft Office Suite
