Job Summary

A company is looking for an Episodic Care Manager to support members with short-term behavioral health or medical needs in a fully remote role.

Key Responsibilities

• Manage short-term member cases by creating tailored care plans
• Collaborate with various healthcare facilities and providers for care transitions
• Evaluate clinical information to determine appropriate levels of care

Required Qualifications

• Active clinical license (LPN, RN, LCSW, LPC, LCPC, or equivalent)
• Experience with psychiatric populations, especially in adolescent care or eating disorders
• Background in inpatient care, utilization management, or residential treatment settings
• Ability to interpret clinical documentation and work with multidisciplinary teams
• Familiarity with utilization review tools like MCG or InterQual
