Licensed Group Support Facilitator

Job Summary

To support a groundbreaking Intensive Cardiac Rehab program, the part-time Licensed Group Support Facilitator will lead group sessions, deliver high-quality patient care, and collaborate with a transdisciplinary team in a remote setting.

Key responsibilities

• Lead group support sessions for cardiac rehab patients according to clinical protocols
• Provide education and support to patients, empowering them to take ownership of their rehabilitation
• Collaborate with technology teams to ensure a smooth patient experience and actively engage in a transdisciplinary team approach

Required qualifications

• Minimum 2+ years of relevant clinical or facilitation experience in a behavioral health, cardiopulmonary, or chronic disease setting
• Licensed clinical social worker, licensed professional counselor, or other mental health professional
• Master's Degree required in social work, counseling, or a related mental health field
• Strong proficiency working with mobile technology to assist patients with digital health programs
• Great interpersonal skills that foster a caring environment for patients
