Job Summary

A company is looking for a Senior Analyst, Health Coach Wellness - Remote.

Key Responsibilities

• Conduct assessments of members' health status to improve medication adherence and health outcomes
• Utilize motivational interviewing to engage members and promote behavior changes for optimal health
• Develop proactive action plans to address health issues and enhance overall wellness through program integration

Required Qualifications

• 1-3 years of experience with Microsoft Office
• 3-5 years of practice experience in areas such as Renal, Diabetes, Hypertension, or Primary Care
• Experience in Managed Care and Medicare
• Associates Degree or equivalent experience
