Licensed Health Coach LVN

Job Summary

Providing one-on-one education on chronic conditions and wellness, the fully remote Licensed Health Coach LVN will establish health improvement plans and personalized goals for clients while supporting behavior changes to enhance their health status and quality of life.

Key responsibilities:

• Establish health improvement plans and personalized goals based on member specifications
• Support customers in achieving health goals through behavior changes to improve health status
• Communicate results of reviews and ensure accurate documentation of clinical decisions

Required qualifications:

• Valid, current LPN/LVN license issued by the state of residence
• Experience in customer service or engagement coaching
• Ability to work independently and complete assigned work accurately and timely
• Familiarity with CMS and state regulatory requirements
• Proficiency in Microsoft Teams and Office applications
