Job Summary
To support care management and quality initiatives, the full-time Licensed Health Outcomes Pharmacist will provide clinical pharmacy expertise, conduct medication reviews, and collaborate with interdisciplinary teams to optimize medication use and improve health outcomes for members with chronic or complex conditions in a remote setting.
Key responsibilities
- Provide evidence-based clinical recommendations to optimize medication use for members with complex conditions
- Conduct medication reviews, identify drug therapy problems, and collaborate with providers on appropriate interventions
- Support initiatives related to HEDIS, CMS Star Ratings, and other quality performance metrics involving medication use
Required qualifications
- Current pharmacist license as required by state board where services will be provided
- Three years of experience in a managed care, health plan, or population health environment
- Three years of proven experience with quality measurement programs including HEDIS and Medicare Star Ratings
- Three years in clinical pharmacy practice
- Knowledge of Medicare and Medicaid pharmacy benefit regulations
