Licensed Home Care Consultant

Job Summary

A company is looking for a Home Care Clinical Consultant I.

Key Responsibilities

• Initiates and maintains professional contacts with hospitals, physicians, and healthcare facilities to develop home care referrals
• Coordinates communication between referral sources and home care agencies to facilitate scheduling of services
• Expands the referral network by establishing relationships and conducting educational presentations on home care services

Required Qualifications

• Bachelor's degree preferred
• 5-7 years of experience, with 4 years as a licensed practitioner preferred; experience in a home care setting is strongly preferred
• Valid LPN, PTA, OTA, or Social Work license in the District of Columbia, Maryland, or Virginia, as required based on work location
• Valid driver's license with access to transportation
• Must be insured for personal injury and property damage liability as per state requirements
