Job Summary

Providing in-home health evaluations, the part-time Licensed In-Home Physician will engage with patients to conduct comprehensive assessments, including medication reviews and physical evaluations, while working remotely and traveling locally as needed.

Key responsibilities

• Conduct in-home health evaluations and connect one-on-one with patients to address health questions
• Utilize an iPad and clinical workflow to perform evaluations and provide diagnostic and preventive services
• Collaborate with Signify Health to fulfill the requirements of the clinician role and provide connections to appropriate care

Required qualifications

• Active, unrestricted medical license in the coverage area
• Multi-state licenses preferred
• MD or DO degree
• Experience in providing patient-centered care
• Ability to perform duties in diverse home environments
