Licensed Intake Coordinator

Job Summary

A company is looking for an Intake Coordinator in Care Management.

Key Responsibilities

• Provide initial outreach, engagement, and screening of members for complex case management referrals
• Research and provide clinical resource information to assist case managers and engagement specialists
• Educate members on case management programs and assist in accessing care and community resources

Required Qualifications, Training, and Education

• Bachelor's degree in social services, human services, behavioral sciences, or a licensed allied health field (Required)
• 3 years of clinical or healthcare experience (Required)
• Motivational interviewing experience is preferred
• Prior Mental Health/Social Work experience is preferred
• Licensed as a Licensed Practical Nurse (LPN) or a Certified Counselor (CC) is preferred
