Job Summary
A company is looking for a Journey Patient Specialist to join their team.
Key Responsibilities
- Develop and process journey referrals while ensuring accurate documentation and workflow processes
- Manage chart closure processes and provide documentation as requested
- Assist other Journey team members with telephonic patient contact across multiple branches as requested
Required Qualifications
- High School diploma/GED
- Previous Journey Counselor experience or Hospice related experience required
- Knowledge of medical terminology required
- Proficient in Microsoft Office Suite including Teams
- Must have and maintain in good standing professional license, certificate, or registration, as applicable
