Licensed Journey Patient Specialist

Job Summary

A company is looking for a Journey Patient Specialist to join their team.

Key Responsibilities

• Develop and process journey referrals while ensuring accurate documentation and workflow processes
• Manage chart closure processes and provide documentation as requested
• Assist other Journey team members with telephonic patient contact across multiple branches as requested

Required Qualifications

• High School diploma/GED
• Previous Journey Counselor experience or Hospice related experience required
• Knowledge of medical terminology required
• Proficient in Microsoft Office Suite including Teams
• Must have and maintain in good standing professional license, certificate, or registration, as applicable
