Licensed Mental Health Coordinator

Job Summary

A company is looking for a Population Health Coordinator - Mental Health.

Key Responsibilities

• Manage data and registries for special populations and facilitate care coordination among team members
• Provide direct member care through outreach, screenings, and education on health-related issues
• Support pilot projects and administrative tasks as delegated by leadership

Required Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with relevant training for medical focus, or a licensed clinical social worker for mental health focus, or a certified peer/community health worker for social care focus
• At least 5 years of experience in direct patient care or supervision in relevant fields
• Bachelor's degree in Nursing preferred for medical focus roles
• Strong understanding of care coordination, case management, and healthcare regulations
• Proficiency in EMRs and documentation tools, with the ability to analyze data and outcomes
