Licensed Mom and Baby Care Manager

Job Summary

A company is looking for a Mom and Baby Care Manager.

Key Responsibilities

• Collaborate with an inter-disciplinary care team to improve member care and outcomes
• Develop and update individualized care plans based on member needs and preferences
• Facilitate communication and coordination among members, providers, and community resources

Required Qualifications, Training, and Education

• Nursing degree from an accredited program or Bachelor's degree in a healthcare field is required
• Advanced degree associated with clinical licensure is preferred
• Minimum of three years of experience in nursing, social work, or healthcare-related fields is required
• Three years of Medicaid and/or Medicare managed care experience is preferred
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor is required
