Licensed Mom and Baby Care Manager

Job Summary

Licensed Mom and Baby Care Manager, a full-time salaried position, is responsible for collaborating with an inter-disciplinary care team to improve member health outcomes through care coordination, assessments, and the development of individualized care plans.

Key Responsibilities

• Engage members and their support systems through assessments and care planning
• Facilitate inter-disciplinary care team meetings and coordinate communication among team members
• Monitor and evaluate member progress and satisfaction with care plans

Required Qualifications, Training, and Education

• Bachelor's degree in a health care field with three years of relevant experience required
• Advanced degree associated with clinical licensure preferred
• Minimum of three years of experience in social work, counseling, or health care profession required
• Three years of Medicaid and/or Medicare managed care experience preferred
• Current unrestricted clinical license in state of practice as a Social Worker or Professional Clinical Counselor required
