Job Summary

A company is looking for a Centralized Health Guide who will lead the medical aspects of virtual care for individuals with serious mental illness.

Key Responsibilities

• Perform virtual whole-person clinical assessments and manage treatment plans for individuals with serious mental illness
• Provide urgent-care treatment and transition of care assessments, ensuring exemplary customer service
• Guide and support in-person health guides and assist with implementing new interventions and work processes

Required Qualifications

• 3+ years of clinical care experience as a Nurse Practitioner, preferably in primary care or emergency settings
• Experience working with populations facing behavioral health and/or substance use disorder challenges
• Willingness to become certified in psychiatric nursing (on the RN level)
• Licenses in FL, MI, OH, VA, WA, or willingness to obtain licenses in these states
• Above average comfort with technology and a desire to perform data-driven care
