Licensed Nurse Practitioner

Job Summary

To support a growing clinical team, the fully remote Licensed Nurse Practitioner will manage patient cases, medication workflows, and care coordination while collaborating with a multidisciplinary team to deliver high-quality, evidence-based care.

Key responsibilities

• Manage patient cases and clinical workflows, providing guidance to support staff and escalating complex cases as needed
• Oversee medication management, including refills and treatment optimization, while addressing patient concerns and collaborating with providers
• Conduct virtual visits and asynchronous care, developing individualized treatment plans for obesity and chronic disease management

Required qualifications

• Master's Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program
• Active RN and Nurse Practitioner licensure in good standing
• Board certification through ANCC or AANP
• Minimum of 2 years of experience in primary care or a related specialty
• Experience with EMR/EHR systems, preferably Athena
