Job Summary

Leading and mentoring a team of 10-12 nurse practitioners, the full-time Licensed Nurse Practitioner Leader will drive team performance, support professional development, and ensure exceptional care delivery while managing a small panel of patients remotely.

Key responsibilities

• Lead, coach, and develop the clinical team to meet operational and engagement goals
• Monitor and enhance team performance through quality oversight and actionable feedback
• Identify best practices and operational improvements to advance care delivery and organizational initiatives

Required qualifications

• NP degree with at least one active state medical license
• 2+ years of direct people management experience, including coaching and performance management
• Demonstrated success in leading teams to achieve clinical and engagement goals
• Experience interpreting performance data to drive accountability and improvement
• Comfortable managing a panel of patients with diabetes, pre-diabetes, and obesity
