Licensed Nurse Supervisor

Job Summary

To support a growing team, the full-time Licensed Nurse Supervisor will manage Care Navigators conducting chronic care management and remote patient monitoring programs, fostering patient success and team engagement in a remote setting.

Key Responsibilities:

• Championing a culture of patient-centeredness and support among Care Navigators
• Empowering Care Navigators by providing tools, resources, and coaching to enhance patient management
• Monitoring performance and conducting regular one-on-one meetings to ensure team success and retention

Required Qualifications:

• LPN/LVN or greater certification
• 5+ years of experience in primary or specialty care practice, home care, or chronic care management
• Proven supervisory experience, preferably in a remote setting
• High computer literacy and comfort with digital health tools
• Strong interpersonal skills with a passion for coaching and mentoring staff
