Licensed Nutritional Support Coordinator

Job Summary

To support patients with chronic conditions, the full-time Licensed Nutritional Support Coordinator will provide remote nutritional education, conduct outreach, and collaborate with clinical teams while managing a panel of 300-350 patients.

Key responsibilities

• Manage monthly nutritional outreach and conduct 50-60+ daily phone calls with patients to provide tailored nutritional care plan support
• Educate patients on nutrition concepts and reinforce provider-directed nutritional guidance to enhance adherence to care plans
• Collaborate with clinical teams to address nutritional needs and escalate patient concerns as necessary

Required qualifications

• Minimum 2+ years of experience in a healthcare role such as care coordination, health coaching, or as a Medical Assistant with a nationally recognized certification
• Active Licensed Practical Nurse or Licensed Vocational Nurse license with a multi-state compact license
• Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience
• Experience with Electronic Medical Records (EMR) and computer navigation
• HIPAA-compliant home office setup with required internet capabilities
