Licensed Nutritional Support Coordinator

Job Summary

To support patients with chronic conditions, the full-time Licensed Nutritional Support Coordinator will provide nutritional education and care coordination remotely, managing a panel of 300-350 patients through outreach and tailored health coaching.

Key responsibilities

• Conduct monthly nutritional outreach and support through 50-60 daily phone calls with patients
• Educate patients on nutrition concepts and reinforce provider-directed dietary guidance
• Collaborate with clinical teams to address nutritional needs and escalate patient concerns as necessary

Required qualifications

• Minimum 2+ years of experience in a healthcare role, such as care coordination or health coaching, with a nationally recognized medical assistant certification or active LPN/LVN license
• At least 2+ years of clinical nutrition counseling or dietary care coordination experience
• 2+ years of experience with Electronic Medical Records (EMR) and computer navigation
• Demonstrated ability to educate patients and simplify complex nutritional concepts
• HIPAA-compliant home office setup with minimum internet speeds of 50 Mbps download / 5 Mbps upload
