Licensed Nutritionist

Job Summary

Delivering member-centered nutrition support, the full-time Licensed Nutritionist will provide nutrition education, develop tailored meal plans, and collaborate with healthcare teams to enhance chronic disease management, all while working remotely.

Key responsibilities

• Provide nutrition education via phone and digital platforms for members with chronic conditions
• Develop culturally appropriate meal plans and dietary recommendations based on members' health conditions and preferences
• Collaborate with case managers and other healthcare professionals to align care plans and optimize lifestyle management

Required qualifications

• Bachelor's degree in health education, health promotion, food science, nutrition, or a related health program
• 3-5 years of experience working with individuals with chronic conditions
• Demonstrated ability to create culturally sensitive nutrition plans
• Certifications in Lifestyle Management and/or Wellness Coaching strongly preferred
