Job Summary

Required Qualifications

• Minimum of 2 years of experience in palliative or hospice care
• Proficiency in leading goals of care conversations and advanced care planning
• Willingness to obtain a certified hospice and palliative social work credential within one year
• Comfortable with technology and working in a fast-paced, remote environment
