Job Summary

A company is looking for a Patient Navigator (Remote).

Key Responsibilities
  • Provide personalized support to patients, caregivers, and family members concerning breast cancer
  • Identify and address barriers to care while facilitating timely access to resources and support
  • Document case interactions and engage in ongoing professional development to enhance support capabilities
Required Qualifications, Training, and Education
  • Minimum of 2 years of experience as a patient navigator, case manager, or social worker in oncology or community health
  • Knowledge of social determinants of care and their impact on patient access and health outcomes
  • Experience with de-escalation techniques and providing emotional support in high-stress situations
  • Bachelor's degree in Social Work, Public Health, Psychology, Human Behavior, or a related field is preferred
  • Bilingual in English and Spanish is a plus; experience with Salesforce or similar databases is desirable
