Job Summary
A company is looking for a Patient Navigator (Remote).
Key Responsibilities
- Provide personalized support to patients, caregivers, and family members concerning breast cancer
- Identify and address barriers to care while facilitating timely access to resources and support
- Document case interactions and engage in ongoing professional development to enhance support capabilities
Required Qualifications, Training, and Education
- Minimum of 2 years of experience as a patient navigator, case manager, or social worker in oncology or community health
- Knowledge of social determinants of care and their impact on patient access and health outcomes
- Experience with de-escalation techniques and providing emotional support in high-stress situations
- Bachelor's degree in Social Work, Public Health, Psychology, Human Behavior, or a related field is preferred
- Bilingual in English and Spanish is a plus; experience with Salesforce or similar databases is desirable
