Job Summary

Key Responsibilities

• Perform and write comprehensive medication reviews and action plans for members
• Drive medication interventions to ensure adherence to evidence-based guidelines
• Collaborate with clinical staff and participate in interdisciplinary care team meetings

Required Qualifications

• Bachelor's degree in pharmacy or PharmD
• Active pharmacist license in the state of position
• 3+ years of experience as a practicing pharmacist in a managed care or clinical environment
• PGY1 residency in ambulatory care or related clinical field is preferred
• Board certification in pharmacotherapy or MTM is desired
