Licensed Population Health Coordinator

Job Summary

A company is looking for a Population Health Coordinator.

Key Responsibilities

• Manage data and registries to support care coordination and quality improvement initiatives
• Coordinate care for complex members and assist in training staff on the Collaborative Care Model
• Provide direct member care, including outreach, assessments, and education on health and recovery options

Required Qualifications, Training, and Education

• For Medical Focus: RN or LPN (Maine only) with SAMHSA training and 5 years of experience in addiction medicine or behavioral health
• For Mental Health Focus: Licensed clinical social worker with 5 years of experience in mental health or integrated behavioral health
• For Social Care Focus: Current social worker or certified peer/community health worker with 5 years of experience in case management or peer services
• Access to reliable internet and telephone services for remote roles
• Must meet pre-employment requirements including background screening and possible health screenings
