Licensed Population Health Pharmacist
Job Summary
A company is looking for a Population Health Clinical Pharmacist.
Key Responsibilities
- Conduct telephonic outreach to patients to identify and address medication adherence barriers
- Complete comprehensive medication reviews and provide evidence-based recommendations to optimize therapy
- Lead training sessions and participate in strategy meetings to achieve quality measure goals
Required Qualifications and Education
- Licensed pharmacist in good standing in the state of residence
- Minimum of 2 years of experience in medication therapy management or related fields
- Familiarity with CMS STARS and medication adherence strategies
- Strong working knowledge of MS Office and Google documents/spreadsheets
- US work authorization
