Licensed Practical Nurse Advocate

Job Summary

As a full-time Licensed Practical Nurse Advocate, the successful candidate will provide non-clinical navigation support to Medicare patients, helping them understand care plans, schedule appointments, and connect with community resources while working remotely as an independent contractor.

Key responsibilities

• Serve as the primary point of contact for patients after their intake visit
• Assist patients in understanding and acting on their care plans, including scheduling and provider communications
• Document all patient interactions and support activities accurately and on time

Required qualifications

• Active, unrestricted LPN/LVN license in good standing
• 2+ years of patient- or community-facing support experience
• Working knowledge of the U.S. healthcare system and Medicare
• Ability to manage a caseload independently and maintain strong documentation
• Reliable computer, internet, and a quiet space for patient calls
