Licensed Practical Nurse Care Coordinator

Job Summary

A company is looking for a High Risk Care Coordinator who is a Licensed Practical Nurse (LPN) dedicated to supporting high-risk patients through compassionate communication and care management.

Key Responsibilities

• Conduct regular outreach to high-risk patients to manage care and reduce gaps in treatment
• Support the development and refinement of a new high-risk patient management program
• Coordinate medication management and address social determinants of health impacting patient care

Required Qualifications

• Completion of an accredited practical nursing (LPN) program with at least two years of nursing experience
• Current LPN license in good standing in applicable state(s)
• Experience in care coordination or chronic disease support preferred
• Proficient in EHR documentation and care management tools
• Experience engaging patients proactively through structured outbound calls
