Licensed Practical Nurse LPN

Job Summary

To support primary care practices, the full-time Licensed Practical Nurse (LPN) will manage pre-visit planning, patient communications, and medication requests in a fully remote capacity, enhancing operational efficiency and patient access.

Key responsibilities

• Review and process pre-visit information to facilitate efficient visit preparation
• Manage and triage incoming patient portal messages to appropriate team members
• Collaborate with clinical staff and providers to streamline communication and reduce delays in care

Required qualifications

• Graduate of an approved LPN school of nursing
• Two years of ambulatory nursing experience preferred
• Current LPN licensure in the relevant state or multistate license under the NLC agreement
• Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification required
• Clearances: Act 33, Act 34, and Act 73 FBI Clearance with renewal
