Job Summary

A company is looking for a Transition of Care Associate Licensed Practical Nurse - Remote.

Key Responsibilities

• Coordinate care for members experiencing significant health changes, ensuring seamless transitions to next care settings
• Complete post-discharge assessments, medication reconciliation, and monitor members for alerts or changes in condition
• Facilitate interdisciplinary care team meetings and provide clinical assistance to address members' health and social needs

Required Qualifications, Training, and Education

• 2+ years of Licensed Practical Nurse (LPN) nursing experience
• Active and unrestricted compact LPN licensure in state of residence
• High School diploma or equivalent; must be an LPN
• Preferred: 3+ years of LPN nursing experience and discharge planning experience
• Ability to multitask and adapt to a fast-paced environment
