Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with a Compact/Multistate License.

Key Responsibilities

• Serve as the primary contact for patients on the RPM365 platform
• Assist with device setup and app connectivity while engaging with patients to ensure adherence to care plans
• Notify healthcare providers of critical alerts and attend virtual team meetings for updates

Required Qualifications

• Active nursing license that is a compact/multistate
• Knowledge of chronic conditions and related monitoring devices
• Proficiency with technology, computer platforms, and smartphones
• Independently motivated and comfortable spending the majority of the workday engaging with patients over the phone
• Experience in telehealth, patient monitoring, education, or customer service is preferred
