Key Responsibilities

Evaluate member needs through clinical post-discharge assessments and medication reconciliation
Develop care/service plans in collaboration with healthcare teams to support member transitions
Facilitate education and resources for members and caregivers to enhance quality of care

Required Qualifications

Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
4 - 6 years of related experience
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
