Job Summary

A company is looking for a Social Work Case Manager.

Key Responsibilities

• Establish trust and build strong relationships with patients through telephonic and virtual communication
• Collaborate with the interdisciplinary team to ensure cohesive, patient-centered care for high-needs patients
• Assist patients in navigating social barriers and facilitate long-term care planning discussions

Required Qualifications, Training, and Education

• Master's degree in social work and active state license
• Minimum of 3 years of experience in medical social work, case management, or care coordination
• Bilingual in Spanish preferred
• Health insurance experience preferred (Medicare Advantage a plus)
• Experience working with the senior population is a plus
