Licensed Social Work Case Manager

Job Summary

Supporting a multidisciplinary team, the full-time Licensed Social Work Case Manager will coordinate care plans for patients with complex chronic conditions, addressing their socioeconomic and psychosocial needs while working remotely.

Key responsibilities:

• Establish trust and build strong relationships with patients through telephonic and virtual interactions
• Collaborate with the interdisciplinary team to provide cohesive, patient-centered care for high-needs patients
• Facilitate conversations about long-term care planning and assist in navigating complex family/support issues

Required qualifications:

• Master's degree in social work and active state license
• Minimum of 3 years of experience in medical social work, case management, or care coordination
• Bilingual in Spanish preferred
• Experience working with the senior population is a plus
• Health insurance experience preferred (Medicare Advantage a plus)
