Job Summary

A company is looking for a Social Worker to join their care management team.

Key Responsibilities

• Partner with RN case managers to assess member needs and develop individualized care plans
• Research and connect members to community resources addressing social, financial, housing, and safety concerns
• Support members through telephonic or electronic communication, promoting engagement and self-advocacy

Required Qualifications

• Active, current, and unrestricted LSW, LMSW, LCSW, or LISW in state of residence
• 2+ years of experience in medical social work or case management
• Master's degree in Social Work
