Job Summary

A company is looking for a Remote Social Worker.

Key Responsibilities

• Conduct comprehensive psychosocial assessments and provide grief and bereavement support to patients and families
• Participate in interdisciplinary team meetings to discuss patient care plans and provide medical social work case management
• Assist patients and families in accessing community resources and navigating social and healthcare systems

Required Qualifications

• 2+ years of healthcare social work experience
• MSW in Social Work or BSW with equivalent experience
• Experience in hospice, palliative care, oncology, or home health
• Palliative care certification is highly preferred
• Bilingual (English/Spanish) is preferred
